Mandatory Requirement: Candidates must present all original documents along with 2–3 sets of self-attested photocopies at the time of verification.
1. Identity & Application Related Forms
- UPSSSC PET Scorecard: Valid UPSSSC PET scorecard as prescribed in the official notification.
- ANM Main Exam Admit Card & Application Form: Printed copy of the online application form submitted during registration along with the DV Call Letter/Hall Ticket.
- DV Forms (Detailed Application Form): Downloaded Forms I, II, III, and IV (Samiksha Adhikari Forms) from the official website (
upsssc.gov.in).
- Identity Proof: Original Aadhaar Card, PAN Card, or Voter ID Card.
- Passport-Sized Photographs: 4 to 6 recent colored passport-sized photographs.
2. Educational & Professional Qualification Certificates
- Class 10th (High School): Mark sheet and Passing Certificate (serves as proof of Date of Birth).
- Class 12th (Intermediate): Mark sheet and Passing Certificate.
- ANM Diploma/Certificate: Mark sheets for all years/semesters and the passing certificate of the 2-year Auxiliary Nurse Midwife (ANM) course.
- UP Nurses & Midwives Council Registration: Valid and active registration certificate issued by the UP Nurses & Midwives Council.
3. Reservation & Domicile Certificates (If Applicable)
- Domicile Certificate: Proof of permanent residency in Uttar Pradesh issued by a competent authority.
- Caste Certificate (SC / ST / OBC): Issued by a competent authority. Note: For female candidates, the caste certificate must strictly be issued using the father's name.
- EWS Certificate: Valid Economically Weaker Section certificate for the relevant financial year.
- Horizontal Reservation Certificates: Certificates for Dependents of Freedom Fighters (DFF), Persons with Disabilities (PwD), or Ex-Servicemen (ESM).
4. Experience & Other Certificates
- Contractual Work Experience Certificate (If Applicable): Work experience certificate issued in the prescribed format for service as an ANM under the National Health Mission (NHM) to claim bonus marks/weightage.
- No Objection Certificate (NOC): Required if currently employed in a government or semi-government organization.
- Character Certificate: Issued by the head of the last attended institution or a Gazetted Officer.
